Vivid Aesthetics Policies & Procedures
Your appointment is very important. We understand that sometimes scheduled adjustments are necessary. Therefore, we respectfully request at least 24 hours’ notice prior to your scheduled appointment time for cancellations or rescheduling of appointments.
Please notify Vivid Aesthetics by email vivid.aesthetics.info@gmail.com if your cancellation is outside of our normal business hours or you’re unable to reach us by phone.
Deposits are 50% of the service and NON-REFUNDABLE. Deposits are due at the time of booking. The remaining balance is due at the time of your scheduled appointment.
You may only reschedule your appointment ONCE using the same deposit.
ANY APPOINTMENTS CANCELED/RESCHEDULED OR CHANGED WITHOUT 24 HOURS' NOTICE WILL RESULT IN AN ADDITIONAL $50 CHARGE. ALL “NO SHOWS” WILL BE CHARGED 100% OF THE RESERVED SERVICE AMOUNT.
Please be on time! The appointment will be canceled after 15 minutes late and you will be charged for the full service. If you are running late please contact me ASAP to prevent your appointment becoming a “NO SHOW”. Please note that although we may be able to wait for you, your session time may be compromised due to bookings back to back.
We recognize the time of our clients and specialists at Vivid Aesthetics is valuable and have implemented this policy for this reason. When you miss an appointment, not only do we lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time.
Please remember that it is your responsibility to remember your appointment dates and times in order to prevent any missed appointments which result in a cancellation fee. Not receiving an electronic notification of your appointments from Vivid Aesthetics is not sufficient reason to miss an appointment if the original confirmation notification was received timely.
It is mutually understood that if a cancellation is due to circumstances beyond any of our control, such as power outage, unfortunate incidence, illness, or weather related that requires you or us to have to cancel or be closed during regular business hours, we will reschedule your existing appointment and no discount or rescheduling fee will apply.
I have read and understood the cancellation policy and agree to abide by the above conditions and authorize Vivid Aesthetics to charge the card on file if need be.

I understand Vivid Aesthetics does not treat medical conditions nor does it claim or guarantee to treat or relieve any medical conditions.

Body sculpting and body therapy should not be used on those with cardiac issues.

Body sculpting and body therapy should not be applied across or on the thoracic cavity.

Body sculpting and body therapy should not be applied over carotid sinus nerves.

Body sculpting and body therapy should not be applied over inflamed, infected, or swollen areas of the skin.

I understand that any procedure involves risk. On a rare occasion risks may include redness, swelling, bruising, irritation, skin reaction, or increased heart rate.
I have been honest and forthright about my medical history, and am healthy to use the equipment required for Lipo Cavitation, Radio Frequency for skin tightening and Vacuum Therapy. I am not pregnant, nursing, have any metallic implants (including a pacemaker), nor any other diseases or conditions that may be negatively impacted by the body contouring devices.
I have, to the best of my knowledge, given an accurate account of my medical history including all known allergies or prescription drugs or products I am currently ingesting or using topically.
I understand each person has a different response to the body sculpting and body therapy treatment. The risks, benefits, and possible results have been explained to me. I have been provided the opportunity to ask questions and received satisfactory responses.
I voluntarily provide my consent to partake in the body sculpting and body therapy treatment. Should any pain or discomfort occur, I will immediately notify Vivid Aesthetics, who performed my procedure and treatment. I will not hold Vivid Aesthetics liable for any irritation or non-desired effects of the body sculpting and body therapy treatment.
It is critical that the dietary and lifestyle changes are made to help support the goals of treatment. To ensure optimal results, we highly suggest drinking lots of water (before and after treatment) brisk exercises (walking, swimming, jogging or aerobics) for a minimum of 30 minutes daily and eating nutritious foods.
I consent to the taking of photographs and authorize the use of them for results, education, and promotion.
This agreement will remain in effect for the procedure and all future follow-ups conducted by Vivid Aesthetics.. I read English and understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I am over 18 years of age and consent to the agreement and to the body sculpting and body therapy treatment procedure.
